Filling in your portfolio – what makes a good entry?

Remember that little things mean a lot to patients, and that you are aiming to make life easier for staff as well as patients.

  • Start with what you are already doing. Think back to changes you have made over the last year or two, and put those on.
  • When you are producing information for patients, ask yourself some questions. How you know it's what patients want to know? Are patients understanding the information they are being given?
  • If you are describing something standard, like a survey you have to do, think about what you have done with the results. Do you feedback to staff? Have you made any changes? Have you let patients know what they are? Or why changes cannot be made?
  • Some entries may meet more than one criterion. Put the evidence under one with an asterisk and note to every relevant heading.
  • If you aren't sure where evidence should go, don't worry. The most important thing is to put it on the portfolio. We will get back to you with feedback.
  • Make the entries as useful to you as you can. Would a new staff member find your entry meaningful? Could you use it in your appraisal?
  • You might like to use the discussion area to make notes of ideas, or things you are planning but which aren't operational yet, or to comment to 4Ps about the portfolio or with questions you have.
